Continuity of Care Through the Inpatient Advanced Practice Provider Program

In a continuing effort to provide exceptional care to our patients undergoing hospital treatment, University Cancer and Blood Center (UCBC) is proud to offer its Inpatient Advanced Practice Provider (APP) program.

Our dedicated Inpatient APP staff are on-site as personal patient advocates to coordinate care and optimize outcomes for new consults and established UCBC patients receiving care at Piedmont Athens and St. Mary’s Hospitals.

How Does Our Inpatient App Program Achieve Continuity of Care for Patients?

  • Initial consults for a comprehensive understanding of patients’ medical goals and options
  • Consolidated record-keeping to streamline communication between oncologists and referring providers, ensuring continuity of care for patients
  • Coordinated care for a more efficient patient experience decreasing length of stay (LOS)
  • Working closely with Palliative Care and facilitating Goals of Care conversations while the patient is admitted with direct input from the patient’s primary Oncology
  • Personal follow-ups and oversight during each hospital visit to ensure comfort and full understanding of the next steps
  • Direct coordination of care between APP and UCBC physicians in all treatment decisions
